Did you know that one in six adults and 40% of primary care patients suffer from medically unexplained symptoms, chronic functional syndromes, or psychosocial factors responsible for chronic pain? Collectively these are called Psychophysiologic Disorders or PPD. As recent research has confirmed, a trauma-informed approach to diagnosis and treatment can transform these patients from among the most frustrating to the most rewarding and give them a far better chance for a full recovery. This process was described in detail in the previous volume from the PPD Association titled Psychophysiologic Disorders (KDP Publishing, 2019).
A key barrier to implementing this approach is that most medical clinicians lack formal training in assessing for PPD and most mental health professionals lack knowledge of medical diagnoses. This book addresses that blind spot with a table of hundreds of medical diagnoses that are common in PPD patients. The table explains each diagnosis and describes the contribution from PPD without jargon or technical terms. It is based on published evidence and the authors’ experience diagnosing and treating PPD in thousands of patients.
Also included is an introduction to the diagnostic process for PPD that includes the authors’ most recent recommendations. There is also a detailed self-assessment questionnaire for patients that is designed to provide insight into how well their condition fits the biopsychosocial context commonly associated with PPD. As in the companion book Psychophysiologic Disorders, the concepts are practical and can readily be implemented by any healthcare professional.
Learning this approach is more than just clinically effective. As one family physician who learned these concepts said: “It put the joy back into my practice.”

I use this weekly. So helpful of a resource. It’s like having access to Dr. Clarke on hand.

Excellent resource!
As a therapist who specializes in treating PPD / neuroplastic chronic pain, this book is a godsend! I’m constantly referring to it with each new therapy client, most of whom have multiple chronic medical conditions. This excellent guide gives me a snapshot regarding whether a condition is likely to be PPD or if PPD may be contributing factor. This gives me additional confidence, especially with clients who present with a chronic medical condition I’ve yet to treat or have never heard of before. Thank you, Dr. Clarke!!

Accessible. Plain Spoken. Unapologetic. Pragmatic.
I appreciate the unapologetic and descriptive manner and tone. Too often it seems the mere mention of PPD (or it’s analogs) elicits debate, sadly to detrimental effect.Herein the authors moved beyond debate and have thus created a pragmatic tool for physicians, psychologists, and laypeople.I also appreciate the care demonstrated by use of terminology which suggests a degree of probability e.g., likely, contributing, is, isn’t, may, and so forth.

There are better books on the topic
I have the greatest respect for Dr. Schubiner and Dr. Clarke, but this book, with the exception of chapter 2, was basically an excel spread sheet saying every condition was possibly a neuroplastic condition or had a brain-pain component. I firmly believe in the principles of neuroplastic pain (or the old term psychophysiological as they use it) and use it to treat some of my chronic pain patients, but as a physical medicine provider and functional medicine practitioner I have to mention that many of the conditions they blame neuroplastic pain on has known physical mechanisms or cellular health/nutritional causes that are not taught to medical physicians, and definitely not to psychologists. So, I wholly endorse the topic as more and more research is confirming the mind-body connection in disease, and strongly encourage chronic pain patients and all physicians to consider neuroplastic pain as a component of healing, there are better books by Dr. Schubiner and I recommend starting with “The Way out” by Alan Gordon.
